Thigh Pain in Uncemented Hip Arthroplasty



Abstract
Authors reviewed 80 hips of 71 patients of uncemented hip arthroplasties, which were performed from June, 1985 to April, 1992 and could be assessed on the aspect of thigh pain recently to investigate relative clinical and radiographic factors
influencing the incidence of thigh pain and suggestive causes of thigh pain biomechanically.
@ES The results were as followings;
@EN 1. With regard to incidence of thigh pain at last follow up, incidence of thigh pain at one year after operation was maintainded until five years after operation with increases slightly.
2. Incidence of thigh pain in patients with anatomic stem and osteoarthritis was less than those with Harris Galante stem and avascular necrosis or fracture.
3. Incidence of thigh pain was significantly low in patients with no radiolucency on porous pad, presence of bony hypertrophy around the distal portion of stem, peresence of pedestal on the tip of stem and subsidence with less than 4mm
radiographically
and patients with proximal bone resorption had relatively low incidence of thigh pain.
4. The findings that patients with radiolucency on porous pad had siginificantly higher incidence of thigh pain and radiolucency around distal portion of stem did not correlate with incidence of thigh pain and patients withbone hypertrophy around
the
distal portion of stem and pedestal at the tip of stem had siginificantly lower incidence of thigh pain suggest that the cause of thigh pain seems to be instability of stem, especially proximal portion, rather than distal transfer of load.
